Ticket Purchases
Be sure and send a message from the Paypal payment
page designating who the payment is from and what it is for.
Contributor Statements are not required for ticket purchases.
You are paying for dinner and entertainment with your ticket
purchase and the value is equal to the price of the ticket.
Individual Contributor's Statement:
This form is required by Oklahoma law if your contribution exceeds $50.00,
or if multiple contributions have exceeded $50.00. Please download, complete
and sign the form and mail to Pottawatomie County Democrats,
PO Box 3033, Shawnee, Ok 74802.
